So after filling out a D1, application for a paper provisional licence, you're going to need to fill out the D750 now (the Photocard Licence application - remember that?) and thankfully, this is a bit easier to do.
Yup, again you've got to fill in all your personal details and then some similar eyesight queries, but Section 3 is important because here you have to tick a code (which are all explained in the leaflet INS108 that you get with your D750) explaining why you are applying - so read the leaflet carefully and tick the relevant box.
In order for your application to be successful, you have to be able to confirm your identity, and in order to do this, you're going to need some documents.
The DVLA suggest that the best form of ID is a passport - since you won't have to include a signed photograph or have to fill in Section 5 (more of this in a bit), but otherwise you could use your birth certificate, adoption certificate, EC/EEA Identity Card or non UK EC/EEA passport. The DVLA require original documents as well ok? So if you send a photocopy then you'll just have your application sent back to you….very annoying.
So what's this Section 5 about then? Well basically, if you're not using your passport as proof of ID, then you'll have to provide a signed photo - not signed by you but by "someone of good standing" e.g. a doctor, lawyer, police officer, MP etc (I got someone at work to do mine!) But it can't be a relative, and has to be someone who has personally known you for at least two years. When they've (hopefully) signed it for you, they then have to fill in their details to show who they are- so choose wisely!
It's worth just mentioning about the photo as well, because the DVLA are quite strict about what they will accept - so you mustn't wear anything resembling a hat or sunglasses, it must be against a plain background and has to be a passport size picture…..oh, and no posing alright?
And that really is just about it! I hope that's helped you to negotiate the application process, because it is a bit baffling if you're not used to the terminology and layout used in these sorts of forms. I certainly know that I found it really confusing when I looked at them for the first time, but if you go slowly and read the instructions then you should be able to fill them in without many problems - Good Luck!
